This has happened to me and others several times, I do not know what causes this bug, but as a developer, I can tell you that the CS spawn probably has an issue with the height.
Although this is a bug for the SV developers, to fix it on your part, simply reset yourself or walk off of the roof. This is a pretty common bug.
